New Patient Registration

To register at either of the practices, you will need to fill in your medical Card and the receptionist will give you a registration form to complete. The form or card will then be sent to Patient Services who will arrange for your medical record to be transferred from your previous GP. When you register, you will be offered a new patient health check. This helps us to get to know you and any medical needs you may have, before the transfer of your records.

From 1st April 2004 all new patients will be registered on a Practice List rather than on a particular GP's list as previously. This does not affect your right to see a particular doctor.

Please note: When registering it is helpful to bring proof of address e.g. utility bill, bank statement, tenancy agreement and also some photographic identification e.g. passport or driving licence.

The practice takes patients from Walkley, Crookes, Malin Bridge, Wisewood, Stannington, Loxley, and heading out towards Storrs.
